什么是过渡?

All students leave school to create their adult lives. "过渡" describes that critical transformation, 包括教育, 法律信息, 规划, and community connections our special 教育 students need in order to achieve those adult goals.

的 transition process prepares students with disabilities for adult life by focusing on the specific areas of post-secondary 教育, 就业, 社区参与, 在适当的时候, 独立生活能力.

过渡 services are intended to be a coordinated set of activities, 由学校提供, 在必要的时候, 社区机构, to students with disabilities to promote successful movement from high school to adult life. 

过渡 services are part of a results oriented process that is focused on improving both the functional and academic needs of a child with a disability.

Federal rules and regulations require that transition 规划 for students with disabilities begin no later than the first IEP to be in effect when the child is 16, 之后每年更新一次.  然而, it is considered best practice to begin transition 规划 as early as when a student is 14, 在某些情况下更早, as they begin to transition from the middle school to the high school.

Who is Involved in the 过渡 Planning Process?

过渡 Services and measurable post-secondary goals leading to a student’s post-secondary vision, must be identified in a student’s Individualized Education Plan (IEP) , beginning with the first IEP in effect when the student turns 16.  因此, IEP小组, 包括学生, 父母, 老师, 在适当的时候,外部机构, 是否参与过渡计划.  Student and parent participation are integral to the success of achieving positive transition outcomes.

 It is important to keep in mind that transition 规划 should not necessarily be limited to the IEP Team, rather it is a partnership involving students with disabilities, 他们的家庭, school and post-school service personnel, 当地社区代表, 雇主, 以及其他投资利益相关者. Successful transition requires collaboration of many individuals and agencies.  然而, consent to invite outside agencies must be obtained from the parent and/or the student, if they have reached the age of majority, prior to inviting community agency members to the IEP meeting.

 In addition to appropriate measurable post-secondary goals, there must be development of a statement of the transition services (including course of study) that are needed to assist the student in reaching those goals.  的 statement of transition services should relate directly to the students post-secondary goals.

什么是成年年龄?

Under Michigan law a child reaches adulthood upon their eighteenth (18th)的生日.  当他们满18岁的时候, all 教育al decision making rights their 父母(s)/guardian(s) have, 会传染给孩子吗. 

的 school district must discuss the impact of the 权利转让 at least one year before the student’s 18th birthday, with the student and parent(s)/guardian(s).  对一些学生来说, a court may appoint a legal guardian that will continue to have authority to make decisions about their 教育al program.  In this case, 教育al decision making will not be transferred.

学生权利手册 serves as notice to the student and parent(s)/guardian(s) regarding the 权利转让.  当学生满17岁时, they and their parent(s)/guardian(s) must receive notice that rights will transfer when the student turns 18.  当学生满18岁时, they and their parent(s)/guardian(s) must receive another notice that their rights have transferred.  Notice of this 权利转让 will also be given in the IEP paperwork.

毕业后会发生什么?

For a student whose eligibility for school is ending due to graduation or age, a local 教育 agency shall provide the child with a summary of the child’s academic achievement and functional performance, which shall include recommendations on how to assist the student in meeting the their postsecondary goals.  This is known as the Summary of performance.
