Media Release Information

During the uedbet官网郡杯, participants may be photographed or recorded by the media or for uedbet官网ISD marketing purposes. Participants 17 and under must have their parent or guardian grant consent via the form below. Participants who are 18 and older should fill out the form on their own.

I waive and surrender any claim that use of my name, 图像, voice and likeness constitutes an invasion of my privacy/publicity rights.
